Started fast
I used this for a English Pale Ale. It started fast and finished clean.

Broken inner pouch
I received the Wyeast 1469 West Yorkshire Ale Yeast, and the pouch was inflated. i thought "no bid deal, it probably got crushed in packing/ shipping", and I'd just stick it in the refrigerator until brew day. It spent a month in the fridge.
When I removed it to pitch, the interior pouch was improperly sealed (sealed seam on a diagonal to the pouch, incomplete seal). It seems as if the pouch was "activated" on production, due to a packaging error. so while this was mildly frustrating, accidents happen. But it probably should have been caught by NB packers as an inflated, activated pouch, and culled rather than shipped out.
Still, the yeast did perform, albeit with a few headaches and a definite stall. But it recovered. I'm hoping the brew isn't spoiled. it still smells nice, almost ready to bottle.
So this is a mixed review. Potential big props to Wyeast for having a clean facility so that even an activated pouch could survive. Mixed feelings about getting an activated pouch on delivery. And maybe a computer error (more forgivable) or a human error in completing the pouch production with a damaged yeast inner packet.
Still NB is excellent, and this little setback won't stop me from ordering more as needed. Overall an inspiring brew supply house. Keep up the good work!
