Design Pieces fell right off the page

Unfortunately, this is the first time this happened, but many of the design pieces fell right off of the page. I have never had this happen and did not notice until I heat pressed the design and realized parts of the design were missing. At this point, they were mangled and the shirt was ruined. I guess next time I will need to make sure this has not happened. I have just not had to do that before.
