Puckered Stitching

The shirt was made to my specifications, but on the collar there is obvious puckering from the stitching which is annoying. There is also an issue with the fit where the shirt is tight across my back, making the sleeves too short if my arms are in any position but down, and there are irregularities in the weave of the fabric which are visible on both sides.
