DON'T FIT IN SHEATH & HARDWARE FALLING OUT

I love the tool WHEN it works! First of all, they don't fit fully down inside the sheath that it comes with. I inquired about that and sent images but was essentially told that's just how the product comes. So, I cannot secure the tool in the sheath with the strap and snap. Sheath is essentially worthless! Second, the nut keeps falling off. So far, I have been fortunate enough not to loose it as I do love the look of the copper hardware. BUT....I am sure the day will come when I dont notice it has fallen off and its gone forever. Any suggestions how to secure it without impacting the function of the tool?
