Nylon bushings tend to pop out; compressing with vise grips or pliers will allow them to remain in place. Note that the secure end of your safety cable should not be permanently anchored - use wire rope clamps if a loop is employed, as the safety cable will need to be unfastened to replace the extension spring in the future when it breaks again down the road.
